Here is the step-by-step process to apply for an Ayushman Bharat PM-JAY (Pradhan Mantri Jan Arogya Yojana) card in Kerala:

Step 1: Check Your Eligibility

The Ayushman Bharat scheme in Kerala covers poor and vulnerable families identified based on the Socio-Economic Caste Census (SECC) 2011 data. Families listed under the deprivation criteria of D1, D2, D3, D4, D5, and D7 categories in the SECC database are eligible for the scheme. You can check your eligibility by visiting the official website pmmvy.gov.in and entering your Ration Card Number or RSBY (Rashtriya Swasthya Bima Yojana) card number.

Step 2: Gather Required Documents

Once you have confirmed your eligibility, gather the following documents:

Ration card

Address proof (Voter ID, Aadhaar card, etc.)

Family member details (Names, Date of Birth, Aadhaar numbers, etc.)

Mobile number

Step 3: Apply Online

Visit the website pmmvy.gov.in and click on the 'PM-JAY Beneficiary Identification' link. Enter your Ration Card or RSBY card number and verify the family details displayed. If the details are correct, proceed to submit the application. You can also upload the required documents through the online portal.

Step 4: Offline Application

If you face any difficulties with the online application process, you can approach the Akshaya e-Kendras or the local village office in your area. They can assist you in checking your eligibility and enrolling for the Ayushman Bharat PM-JAY scheme. Provide them with the required documents mentioned in Step 2.

Step 5: Ayushman Card Generation

Once your application is processed and found eligible, your Ayushman card will be generated and delivered to your residential address within 60-90 days. The card will have a unique 16-digit identification number and will contain the details of all the family members covered under the scheme.

Step 6: Check Card Status

If you haven't received your Ayushman card within the expected timeline, you can check the status by calling the toll-free numbers 14555 or 1800-425-0660. Provide your Ration Card or RSBY card number, and the customer care executive will provide you with an update on the status of your application and card delivery.

Step 7: Utilize the Benefits

With your Ayushman card, you and your eligible family members can avail cashless treatment worth up to Rs. 5 lakhs annually at any empanelled public or private hospital across India. The card covers secondary and tertiary care hospitalization, including pre and post-hospitalization expenses.

In Kerala, the Ayushman Bharat scheme is implemented in convergence with the state's Karuna Arogya Suraksha Padhati (KASP) scheme. This means that if you are already enrolled in KASP, you will automatically be covered under the Ayushman Bharat scheme as well.

Remember, the Ayushman Bharat PM-JAY scheme aims to provide financial protection and access to quality healthcare for the underprivileged sections of society. Follow the steps carefully, and if you face any issues during the application process, reach out to the concerned authorities or Akshaya e-Kendras for assistance.
