Step 4: Choose the “Self” option if it's for your personal PVC Voter ID card; opt for “Other” if it's for someone else. Enter the Voter ID card number in the EPIC number section and click “Submit.”


Step 5: After clicking submit, verify your name and other information on the screen. If correct, click “OK.”


Step 6: Select "Issue of replacement epic without correction" as the reason.


Step 7: The form will open, and you'll need to enter your Aadhaar card number. Below, click on the self button for your mobile number, then click “Next.”


Step 8: Select the reason for replacement, and if your voter ID is lost, choose the appropriate option. If you select lost, upload a picture of the FIR. Otherwise, choose the second option for reasons beyond control.


Step 9: Enter your location details, including the village name, and provide the CAPTCHA code. Click “Preview and Submit.”


Step 10: In the preview, ensure all details are correct, then click “Submit.” Capture the reference number displayed for future use and click “Download acknowledgment.”


Step 11: Check the status of your application on the homepage by entering your reference number and state. Keep an eye on the status as it progresses from submission to approval. Once approved, your new plastic PVC voter ID card will be delivered to your doorstep within 15 to 20 days.
